What is lower blepharoplasty?
Lower blepharoplasty is a cosmetic surgery procedure, performed by Dr Maxence Thomas at the Clinique Esthétique Aquitaine in Bordeaux, that aims to correct certain signs of aging of the lower eyelid:
→ Bags under the eyes caused by sagging skin and fat accumulation, accentuated by muscle laxity.
→ Lower eyelids wrinkled by excess skin.
→ The hollow of the tear trough is not treated by blepharoplasty, but can be treated at the same time with an injection of fine fat known as microlipofilling or nanofat.
How does the procedure work?
The surgeon can use 2 types of incision, depending on the planned procedure:
- The transconjunctival approach
The incision is made on the inner side of the eyelid, in the conjunctiva. This approach is used when the aim is to remove fat pads.
- The subciliary approach
The incision is made just below the lashes, where the scar is barely visible and easily concealed. This approach is used in particular to remove excess skin.
Both approaches can be combined in the same procedure if several steps are required (removal of fat pads + skin tightening, etc.).

What is the recovery like?
Scars
Transconjunctival scars cannot be seen from the outside (inner side of the eyelid) and subciliary scars are barely visible and easily concealed.
Post-operative course
Stinging eyes, blurred vision or slight itching may be felt temporarily. Eye drops are prescribed for the first few days. Swelling and bruising of the eyelids should be expected for 15 days.
Recovery
10 to 15 days of social downtime are required, but clinical recovery is very quick. The patient can go out during the first few days wearing sunglasses.
What results can you expect?
Lower blepharoplasty treats the entire lower eye area in a single procedure.
→ Barely visible scars
→ No unnecessary invasive steps
→ Respects the normal anatomy of the eyelid
How much does it cost?
From €2,650
A detailed quote is given after the consultation. For a cosmetic procedure, French law requires a cooling-off period of at least 15 days between the quote and the surgery.
FAQ
Depending on the technique used, the scar may be completely hidden inside the eyelid (transconjunctival approach) or hidden just below the lashes (subciliary approach). The skin of the eyelids is very thin, which generally allows the scar to fade quickly until it is almost imperceptible.
Lower blepharoplasty is often performed under local anesthesia, even when combined with upper blepharoplasty. It can also be performed under general anesthesia.
The procedure usually takes 30 to 45 minutes, depending on its complexity.
It is a surgical procedure that is performed on an outpatient basis.
Depending on the case: removal of excess skin, of fat pads, or both at the same time.
It is a procedure with little pain; discomfort is well managed with usual painkillers.
As with any procedure, there are risks, and complications may occur (bleeding, infection, wound dehiscence, scarring problems, neurological disorders, etc.). They are uncommon and can be managed promptly by the surgeon during post-operative check-ups. An information sheet from the French Society of Plastic Surgery will be given to you before the procedure.
Any other questions? Dr Thomas will answer them during the consultation.
